In order to begin the planning, processing and facilitation of your medical travel overseas, it is prudent for the client/patient to formally download and complete the following forms as follows. Please ensure that you provide precise and accurate medical history of your condition for optimum treatment.

CONFIDENTIALITY

Transmission of the patient’s private medical information to a specialist healthcare provider overseas, shall be made only after due consultation with the patient. GHL & L, will safeguard, and protect individual client medical conditions in the strictest confidentiality and privacy.

Form1
(Authorization)

Upon signing this form, i hereby unequivocally authorise Global Health Logistics (Ghana) Ltd to represent me as my medical travel AGENT. I also understand that, GHL & L, have my permission and authority to disseminates my medical history and supporting documentation for my medical treatment specialist, medical surgeons / healthcare providers abroad.

Form 2
( DESIRED TREATMENT)

This for seeks to capture our goals, expectation, and objectives of the treatment you seek to undertake. This information will be transmitted, electronically/by courier, eg as in X-ray reports to a specialist physician/doctor abroad for thorough diagnostic assessment recommendation. At this stage an estimated direct cost of the surgery/ treatment for the procedure will be made available.

Form 3
(Your Bio-DATA)

This form seeks to capture your complete medical history for your doctor abroad. This is important to enable your surgeon to understand your specific care.

Preparing

GHL Suggest that, you prepare yourself for surgery.
To achieve this, we suggest that:

Quit smoking Cigarettes or tobacco
Loose weight (if your overweight)
Show all medications including list, such as aspiring, or arthritis related medications
Consider donating your own blood before the surgery for possible transfusion during surgery

INITIAL COST AT GHL ( GHANA LTD)
  •  Initial consultation fee (Non refundable)
  • Patient / client medical documentation fee
  • Travel assistance, eg Visa /ect preparation and documentation fee
  • Foreign / overseas Doctors appointment and communication fee
  • Communication fee (Telephone / Electronics fee)
  • Local Transport- Ambulance or special transport fee
  • Service charges fees
  • Hotel Booking fee at treatment destination
  • Transport charges, eg Ambulance service from airport to hotel
  • Cellular phone and airtime connection fee immediately and close monitoring and reporting to family members in Ghana in upon arrival.
  • Overseas doctors diagnosis and assessment fees
DESTINATION TREATMENT FEES / COST
  • Hotel accommodation fees – Based on, proximity to the treatment centre / hospital, the number of days and the rate per day
  • Local transportation charges from Hotel to Hospital and from Hospital to Hotel.
  • Cost of Hotel accommodation for accompanying spouse or family relative
  • Cost of return airfare for Ghana to destination for client or accompanying family member
NON TREATMENT AUXILIARY COST
  •  If admitted on hospital bed, the charges of the hospital per day multiplied by the number of hospital bed days.
  • Cost of medications and special hospitalisation (Cost example, laboratory test, for pre and post treatment analysis).
  • Doctors / Surgeon charge for treatment or Operation, based on your medical treatment Requirement.
  • Medical travel insurance in case of any eventuality.
  • A provision for transportation of deceased body and related mortuary expenses upon death.
